Dear Patient:

The physicians and staff of The Heart Group have always taken pride in their adherence to the principle of confidentiality of patient medical records and information. With the passage of the Health Insurance Portability and Accountability Act (HIPAA) of 1996, Congress has mandated that all health care providers formalize their confidentiality policies and inform you of them. In addition, we are required to document our communication of our policies and in certain instances obtain your approval for us to share your information with individuals and organizations outside The Heart Group.

In order to be in compliance with HIPAA, The Heart Group is required to document your receipt of our Notice of Privacy Practices for Protected Health Information. As such, you will be asked to sign the following form upon registration:
